Many of you already know me. My name is Beth Sholom and I am the Owner of Right at Home of Central New Jersey and we serve the communities of Middlesex and Northern Monmouth Counties. I wanted to take a moment to share some things I’ve learned about the challenges of discharge planning for patients at your facilities.
After speaking to many social workers and discharge planners in our area, I made a list of the biggest challenges they’ve told me they face, as they get ready to send a patient home.
· Fast communication and confirmed home care availability. Let’s face it, on a busy floor what you really need to hear is “YES, we will take care of everything, today, within 2 hours.”
· Late afternoon discharges are complicated sometimes. Yes, they can be. But the best home health care agency will say “YES, we can take care of this, even at 5:00 pm” (and deliver on that commitment).
· Friday discharges can also be complicated and you worry that the patient won’t see anyone for hours or even days. Guess what happens when home care doesn’t show up? The patient is re-admitted via the emergency room, correct? You need responsive and fast care, even on Friday evening, Saturday and Sunday.
· No Follow Up: Very few home health care agencies ever follow up to say that the patient is home and doing well. We do and always will.
· As the social worker or discharge planner, you promise the family that everything is going to be ok and that “home” is the best place to recover. And then you worry that the home health care company they choose will be a complete mess and cause the family to have a terrible discharge experience. This leads to … you guessed it …. readmission within 30 days.
After listening to what my friends and colleagues describe as their greatest challenges with discharge planning, we at Right at Home of Central New Jersey decided to address these challenges right away.

· You WILL receive a follow up text or phone call from our staff to let you know that we have been to the client’s home and that everything is ok.
· Our top priority is to be fast, efficient and effective in delivering home care services. We understand the challenges with 30-day readmissions and we do everything we can to make sure the client is safe, comfortable and recovering, not regressing.
My hope is that, by sending you this personal note, you will understand both Right at Home of Central New Jersey’s commitment and my personal commitment to our clients/patients and our commitment to you as a busy discharge planner/social worker.
