RIGHT AT HOME OF CENTRAL NEW JERSEY has partnered with a visiting physician medical group to provide our clients with additional care options. Our partner can provide home visits to our clients offering such services as:

·       Comprehensive annual or quarterly wellness visits

·       Acute/sick patient visits

·       In-home phlebotomy and blood work

·       In-home EKG testing

·       In-Home ABI and spirometry (lung function) testing

·       Environmental and nutritional checks performed by doctors (to check for fall risks items, etc.

·       24-hour access to doctors

·       24-hour patient portal access to your medical records and lab results

·       4 locations located throughout New Jersey open 7 days a week for urgent in-clinic visits

·       Nurse Case Management care plan oversight and telephone access

·       Medication management

·       Medicare-approved social workers available for in-home visits when needed

·       Affiliated in-home specialists providing podiatry, wound care, dentistry, x-ray, ultrasound, physical therapy, occupational therapy and behavioral health therapy as needed

·       Social Services need assessment for such things as Meals on Wheels and grooming services by specialists experienced with working with senior clients

·       The ability to purchase a tablet so that the client can have access to telemedicine (two-way, real-time interactive between the client and the physician). The table also offers our clients the ability to facetime (interactive real-time video chatting) between the client and his/her family. You’ll also be able to purchase and access exercise videos that can be viewed on the tablet.
